Velocity Sports Lab Participation, Liability, & Facility Use Waiver

Activities Covered

Description: This waiver applies to participation in: • Speed, agility, and sports performance classes • Open play and recreational sports activities • Use of equipment, turf, courts, and training areas • Birthday parties, private rentals, and small events

Assumption of Risk

Description: I understand that athletic and recreational activities involve inherent risks, including falls, collisions, equipment failure, illness, and physical injury. I voluntarily assume all risks associated with participation and facility use.

Liability Waiver & Release

To the fullest extent allowed by law, I release and hold harmless Velocity Sports Lab, LLC, including its owners, employees, contractors, and representatives, from any claims or liability arising from participation or facility use, except in cases of gross negligence or intentional misconduct.

Indemnification Agreement

I agree to indemnify and hold harmless Velocity Sports Lab, LLC, and its owners from any claims or expenses resulting from my actions, misuse of the facility, or actions of guests I bring to classes or private events.

Medical Authorization

I authorize Velocity Sports Lab, LLC, its owners, and its employees to obtain emergency medical care for myself or my child if necessary and accept full financial responsibility for any associated costs.

Facility Rules

I agree to comply with all facility rules, including proper use of equipment; no unsupervised activity unless permitted; no alcohol, smoking, or illegal substances; supervision of minors during private rentals; and cleaning/restoring the facility after private events. Velocity Sports Lab may remove any person for unsafe or disruptive behavior.

I understand that this waiver applies but is not limited to all activities listed above; I acknowledge and accept all risks associated with participation; I agree to the liability waiver and release; I agree to indemnify and hold harmless Velocity Sports Lab, LLC, and its owners; I agree to the medical authorization; I agree to comply with all facility rules.

I grant permission for Velocity Sports Lab, LLC to photograph or record the participant for marketing or promotional purposes.(Required)
